What happens to your muscle when you come off a GLP-1?
Stopping is when the muscle question gets loud; ProForta helps you maintain muscle while you lose the weight, measured over twelve weeks, and the 60-second self-check starts the record.
Stopping is a legitimate decision, and the reasons people stop are their own and their prescriber's. What the follow-up data can tell you is what the period after tends to look like: regain is common, and unless the muscle-protecting habits stayed in place, it tends to arrive as fat rather than as the tissue you lost.
People search for this under semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound). The body-composition answer below is the same whichever one you are on, and it is the same again if you are losing weight without any of them.
What the follow-up data shows
The clearest published look is the STEP 1 trial extension, which followed participants for a year after withdrawal. Mean weight loss was 17.3 percent at week 68. A year after stopping, participants had regained about two-thirds of what they had lost, leaving a net loss of 5.6 percent at week 120.
Two things are worth reading in that. The first is that a third of the loss was still there a year later, which is not nothing. The second is that the composition of what returns is a separate question from the amount, and nothing about eating more sends a signal to rebuild lean tissue. Fat comes back readily on its own. Muscle does not.
Why the muscle you kept matters more afterwards
Muscle is metabolically expensive tissue. If you finished a weight-loss window with less of it, you need fewer calories at rest than you did before, which makes the return of an ordinary appetite land harder than it used to. That is the mechanism behind the common experience of regain feeling faster than the loss was.
The corollary is more useful than the warning. The muscle you protected on the way down is the thing still working for you on the way out, and it is the one part of the outcome that is fully within your control.
What carries over
Neither of the two levers depends on the medicine. The 2025 joint advisory treats 1.5 grams of protein per kilogram of fat-free mass a day as the more accurate target, or 80 to 120 grams a day without a scan, and its exercise recommendation is strength training at least three times a week plus at least 150 minutes of moderate aerobic activity. Those numbers are the same on the last week and the week after.
The S-LiTE trial is suggestive here too: over a year, the group that combined exercise with the medicine lost the most weight and the smallest fraction of lean mass, cutting body fat percentage by 3.9 points against 1.7 for exercise alone and 1.9 for the medicine alone. The training half of that combination is the half that does not stop when a prescription does.
What you can check this week
- Keep the strength sessions in the calendar. They are the part of the arrangement that carries over unchanged, and they are the first thing that quietly disappears.
- Recheck your protein target. If you have a recent scan, multiply your fat-free mass in kilograms by 1.5. If not, the advisory's 80 to 120 grams a day holds on the medicine or off it. The 60-second self-check shows both.
- Set a new week zero. Weight, three lift benchmarks, a waist measurement, a chair-stand count. The next phase deserves a baseline as much as the last one did.
- Watch appetite return without reading it as a verdict. Hunger coming back is the medicine wearing off, not a personal failure, and the plan for it is the same plan.
If you are deciding whether to stop
That decision belongs with your prescriber, who knows your situation. Nothing on this page speaks to it. What this page can offer is the one thing worth doing before the decision either way, which is establishing what your muscle is doing right now, so that whatever comes next has a starting point.
Where ProForta comes in
ProForta does not stop being useful when the medicine does. It keeps reading your weight trend, your resistance work, your protein and your recovery on a private node that belongs to you, and keeps giving you one read on whether the muscle is holding. If you leave, the node forgets you on request and the erasure is cryptographic rather than a checkbox. LifeCare.ID® carries the identity and the consent record.
ProForta is a wellness tool, not medical advice. Nothing here speaks to your medicine or your care. Your prescriber stays the decision-maker on both.
Watch it instead of guessing
The ProForta Challenge is twelve weeks with your muscle actually watched while you work on the weight, however you are going about it. It is free, it is a small beta on one private node, and the only person you are ever compared with is you at the start of it.
The self-check runs entirely in your browser and sends nothing anywhere. If you would rather look around first, open the demo, nothing to install and no account to create.

Corrected 8 October 2026: an earlier version of this page attributed a protein minimum of 1.2 grams per kilogram of body weight and a range of 1.6 to 2.2 to the 2025 joint advisory. The advisory does not give those figures; the figures above are its own.
